What are the main tracheostomy emergency

What is a tracheostomy
Opening to the trachea at 2/3 ring
A stoma is made in the trachea whereby this is a another airway to aid breathing
Maybe permanent or temporary
What should you straight away ask for when attending pt’s with a tracheostomy or laryngectomy
Patient passport or care plan = follow it to the letter
- incorporate PTS responsible person ( competent = had training)
How do you suction any secretion in pt with tracheostomy or laryngectomy
- remove inner cannula ( check for blockages)
- replace with non fenestrated one
- pre oxygenate
- measure inner cannula with suction catheter
- insert catheter with ( inner cannula in insitu) upto 2cm past
- suction only when retracting @150-200
- re oxygenation required
What is the most common cause of seizures in children and why
Febrile upto 5 yrs - due to the hypothalamus isn’t full developed = during infection temp sky rockets = seizure
What should you document in any child who has had a seizure
If they were unwell, feverish, and eventually prior = any head injury
Plus general stuff past med ect
What should you never do in pediatric patient who are having a seizure in terms of drugs
- ensure drugs are given correct based on weight/ age
- drugs are given in full ** no titration** ,
- if your giving a drug make sure you still continue even if whilst giving seizure stops
What should you consider if a patient is having a febrile seizure ( post seizure or if in status and you have time )
IV paracetamol
Which types of children should be conveyed if they have had a seizure
- status epilepsy
- ABC problems
- under 2 yrs
- first febrile
- more than one benzo given
