Level 1
Total assist - No response - Complete absence of observable change with all stimuli
Level 2
Total assist - Generalized response - reflex to pain
Level 3
Total assist - Localized Response, response to pain, follows visual stim, may respond to family
Level 4
Max assist - Confused and agitated. alert and increase state of activity, will remove tubes/restraints, decrease short term memory, unable to cooperate with treatment, verbally inappropriate
Level 5
Max assist - Confused and Inappropriate - non agitated and not orientated to person time and place
Level 6
Mod assist - Confused Appropriate, inconsistently oriented, 30 min of activity, vague recognition, supervised for old learning (ADL) Unaware of impairments
Level 7
Min assist - Oriented, new learning possible, over estimates ability, oppositional/uncooperative
Level 8
SBA - Purposeful and appropriate, oriented, depression, easily angered, argumentitive and self centered but able to correct action
Level 9
SBA - Purposeful and appropriate, is able to shifts between tasks, can use a to do list, household work, leisure, ADLs, aware of disabilities, can monitor self appropriateness
Level 10
Mod Independence - Purposeful and appropriate, can handle multiple tasks, carries out steps of task, knows impact of disabilities, knows needs and feelings of others,
