What is the general format of any radio call, and what is the general format of any read back?

A
Who are we talking to? 
Who are we?
What are we?
Where are we?
What other useful info do we possess?
What do we want?

A read back involves:
Any heading, height, speed instructions, pressure settings, clearances, or for any other instructions can use WILCO or RODGER.
Followed by further intentions if needed
Followed by callsign.

A

Callsign,
SID if on a SID,
Current level passing,
PLUS initial level climbing to.

Eg. EZY12AB on a CLN3X, passing 4000ft, climbing 6000ft…
