Quinolones In FluroQuinolone Flashcards
What was the first quinolone
Nalidixic acid
What type of bacteria is nalidixic acid effective against
Gram-negative bacteria but not pseudomonas
What is the mechanism of action of nalidixic acid
- Inhibits DNA Gyrase enzyme
- interferes with replication of bacterial DNA
What are Fluoroquinolones
Synthetic, fluorinated analogues of nalidixic acid
What is the mechanism of action of fluoroquinolones
- Inhibit bacterial synthesis
- inhibit DNA Gyrase, thus blocking DNA replication in gram -ve bacteria
- inhibition of topoisomerase IV in gram +ve bacteria - prevents separation of replicated DNA
Name common flouroquinolones
- Norfloxacin
- ciprofloxacin
- Pfloxacin
- ofloxacin
- levofloxacin
- Gemifloxacin
- moxifloxacin
- Prulifloxacin
- Trovafloxacin
Which fluoroquinolones inhibit the metabolism of Theophylline and warfarin
- Norfloxacin
- ciprofloxacin
- Pfloxacin
The metabolism of which drug does ofloxacin inhibit
Theophylline but to a lesser extend
Which flouroquinolone has a bioavailability of 100%
levofloxacin
Which Bacteria are resistant to ciprofloxacin
Most anaerobes such as bacteroides fragilis, C. Difficile, etc.
What Delays the absorption of ciprofloxacin
Food
What are the adverse effects of fluoroquinolones
- G.I. disturbances: Nausea, vomiting
- CNS effects: headache, dizziness, insomnia, infusion, hallucinations and convulsions
- Hypersensitivity reactions including skin rashes, eosinophilia, photosensitivity, uriticaria
- Tenosynovitis and Can cause tendon rupture in athletes
- Moxifloxacin causes prolongation of QT interval
- Contraindicated in pregnancy and young children
Which drug can cause prolongation of QT interval
Moxifloxacin
What are the contraindications of flouroquinolones
- Pregnancy
- young children
What happens when NSAIDs are used with fluoroquinolones
The NSAIDs may potentiate CNS side effects of florouquinolones - confusion, irritability, and rarely convulsions
