what is pulmonary hypertension
high blood pressure in the blood vessels that supply the lungs
resting mPAP is >25mmHg

mild 25-40
mod 40-55

- pre capillary

- chronic hypoxemia

pathophysiology of pulmonary hypertension
- Reactive pulmonary vasoconstriction in response to hypoxemia (body thinks this is DEAD SPACE)
- increased pulmonary vascular resistance and pressure
- therefore endothelial damage
- leads to → RVH and resp failure
initial symptoms of pulmonary hypertension
EXERTIONAL DYSPNOEA + FATIGUE first
later symptoms of pulmonary hypertension
RV failure signs:
- JVP distension
- peripheral oedema
- cannon A waves
- louder S2 than normal
3 investigations for pulmonary hypertension
- RH catheter - DIAGNOSTIC
- chest x ray
- ECG

Evidence of RA dilation
(‘p pumonale’ - peaked p waves)
how to treat pulmonary hypertension
- Sildenafil (VIAGRA!) - phosphodiesterase 5 inhibitor
- calcium channel blocker - amlodopine
- endothelium 1 antagonists/ prostaglandin analogues
