A

left heart disease

pulmonary vascular disease (idiopathic pulmonary hypertension, pulmonary vasculitis, sickle cell, pulmonary embolism, portal HTN)

hypoventilation (obstructive sleep apnoea, morbid obesity, neuromuscular)
