Cardiogenic (Hydrostatic)
Non-Cardiogenic (wall permeability)
Causes/Findings of Cardiogenic Pulm Edema
Usually caused by left heart failure
Kerly B lines might be seen on CXR (lymphatic swelling)
Causes of Non-cardiogenic Pulmonary Edema
Widespread injury
DOES NOT IMPROVE w/ DIURETICS
Definition of Pulmonary Hypertension
Pulmonary Arterial Pressure >25mmHg
5 Classifications of PHTN
- Arterial PHTN
- Left Heart Disease PHTN
- Lung Disease/Hypoxia PHTN
- Chronic Thromboemoblic Disease PHTN
- Idiopathic
Types of Arterial PHTN
idiopathic, heritable, drugs/toxins, connective tissue disease, portal hypertension
CXR, ECG, Lab findings in acute PE
CXR usually normal!
ECG: right heart stress, tachycardia
Las: high A-a gradient, high D-dimer
Pulmonary Arterial Hypertension Treatment
Pulmonary vasodilators Endothelin R antagonists PDE5 inhibitors (better NO effect) Prostacyclins (dilation) Maybe CaCBs
Pulmonary Vasodilator Use in Pulmonary HTN
DO NOT GIVE W/ LHF PATIENTS (causes P Edema)
