What needs to be monitored in the immediate puerperium?
BP/pulse q 15min, temp q4h. watching for hemorrhage, endometritis, ecclampsia
What happens in early puerperium (24hrs to 1wk)?
cervix closes, lochia, muscles decrease in size
What happens in remote puerperium (6 wks)
uterine involution, lochia ceases, striae fade, hair falls out
How long do mothers stay in the hospital (not including day of delivery)?
48hrs for vaginal delivery, 96hrs for C section
What are minimum caloric requirements for breast feeding
1800 kcal/d
When are the post-partum visits?
7-14 d after c section, 4-6 wks after Vaginal delivery
What are the 3 stages of lactation?
mammogenesis = growth and devlp’t (need estrogen & progesterone), lactogenesis = initiation of milk secretion (need prolactin), galactopoeisis = continued production (needs oxytocin, prolactin)
How many calories are in breast milk?
60 - 75kcal/dL
What are contraindications to breastfeeding?
HIV, breast CA, drugs/etoh, TB or varicella, galactosemia, certain meds, T cell leukemia virus
When should breastfeeding be initiated?
immediately after birth (< 1hr)
What is a complication of untreated mastitis?
abscess
