PTA 2.1 Flashcards
how to establish prognosis for walking abilities?
on day 2 after stroke, assess leg motor function (by using Motricity index) and sitting balance (using Trunk Control Test)
in which case is functional recovery favourable?
25 points on sitting balance item of TCT and mild paresis from motricity index ( 25 or above points)
Chances of regaining independent walking ability are also determined by?
- initially reasonable ADL skill
- younger age
- absence of homonymous hemianopia: visual field defect
- urinary continence
- absence of premorbid limitations of walking abilities and ADLs
prognosis for dexterity is determined how?
to establish we need: capacity for finger extension (Fugly Meyer Assessment, 1 or above points) and shoulder abduction (assessed with Motricity Index, 9 or above points). Best perform 2 days after stroke.
SAFE MODEL
shoulder abduction - finger extension
prognosis for basic ADL activities after stroke
determine Barthel index at the end of 1st week after stroke but best on day 5
which test do you use to determine prognosis for functional changes in walking ability
FAC, score of 3 or more at 6mo
whats motor aphasia called?
broca’s
whats sensory aphasia called?
wernickes
name 3 impairment types you can have (theyre in pairs)
socio emotional
sensori-motor
cognitive neuropsychological
when should you use basic measurement instruments?
- during diagnostics
at the end of 1st week after stroke
3months
5months
end of treatment period
when trasnfering patient to another PT
which test has a sign. ceiling effect?
trunk control test
which basic measurement instrument assesses balance?
berg balance scale
what does berg balance scale measure?
balance
used to objectively determine a patient’s ability (or inability) to safely balance during a series of predetermined tasks
whats the entry criteria to perform 10MWT?
FAC 3, 4 or 5
whats FAC 3?
ambulator, needs supervision but able to move around on level surfaces
to be pedestrian, how fast should your walking speed be?
> 1m/s
in case of shoulder complaints, whats the score you can get on FAT?
max 4 points
whats parkinsons disease
dysfunction of the basal ganglia, caused by degeneration of dopamine-producing cells in the substantia nigra
sequence of functional recovery?
- care for external appearance
- eating
- transfers
- toilet use
- mobility
- undressing/dressing
- showering
- walking stairs
whats prosopagnosia?
cognitive disorder, impaired ability to recognize faces
easy way to remember prognosis for dexterity at 6months after stroke?
SAFE model
shoulder abd (MI >=9)
finger ext (FMA >=1)
whats the TWIST algorithm? explain it
time to walking independently after stroke
1 week post stroke you assess patient’s TCT
if its above 40, within 6 weeks patient will walk independently
if its below 40, measure MRS hip extension - if its 3 or above, patient will walk within 12 weeks. if its below 3, patient will be dependent
after first 3 months, how much % of recovery has taken place?
80%
