what is the most common cause of preterm birth?
spontaneous preterm labour
other common causes are multiple pregnancy, membrane rupture & pregnancy associated hypertension
what are some risk factors for preterm birth?
more than 2 previous preterm births, multiple pregnancy, <6 months between pregnancies, IVF, smoking, alcohol & drugs, multiple miscarriages / abortions
how do you keep preterm infants warm?
put them in a plastic bag immediately after birth and putting them under a radiant heater with a hat on
what are the common problems of prematurity?
temperature control
feeding/nutrition
sepsis
system immaturity/dysfunction - RDS, PDA, IVH, necrotising enterocolitis (NEC)
why is thermal regulation ineffective in preterm infants?
low basal metabolic rate
minimal muscular activity
almost no subcutaneous fat insulation
high ratio of surface area to body mass
what is the difference between early and late onset neonatal sepsis?
EOS is due to bacteria acquired before or during delivery
LOS is due to bacteria acquired after delivery
what organisms cause early onset neonatal sepsis?
group b strep
gram negatives
what organisms cause late onset neonatal sepsis?
coag -ve staph
gram -ves
staph aureus
what is the management of neonatal sepsis?
preventions
hand washing
antibiotics
super vigilant and infection screening
what are the respiratory complications of prematurity?
RDS
apnoea of prematurity
bronchopulmonary dysplasia
what is the primary pathology of RDS?
surfactant deficiency
structural immaturity
what is a secondary cause of RDS?
alveolar damage- formation of exudate from leaky capillaries
what are the clinical features of RDS?
respiratory distress
tachypnoea- grunting, intercostal recessions, nasal flaring, cyanosis
what is the management of RDS?
maternal steroid
surfactant
ventilation
what are the classifications of intraventricular haemorrhage?
Grade 1- 4 classification and outcome:
Grade 1 and 2 : Neurodevelopmental delay up to 20%
-Mortality 10%
Grade 3 and 4: Neurodevelopmental delay up to 80%
-Mortality 50%
