What are the CD prescription requirements
A
- Only valid 28 days
- Total quantity written in both words and figures
- Not a legal requirement but strongly recommended not to exceed 30 days supply
- Schedule 2 and 3 not repeatable

A
- FP10MDA
- For treatment of addiction e.g. buprenorphine, diazepa,, methadone
- Home office licence required for cocaine, diamorphine, dipipanone
- Must specify amount of medicine per instalment
- Must state interval between instalment
- Specificy separately on Rx
- 1st instalment to be dispensed within 28 days

A
- Only schedule 2 and 3
- Must have prescriber identification number
- Original Rx must be submitted to relevant NHS agency
- Identification code assigned to pharmacy for this purpose

What are the requirements of vet CD private prescriptions?
A
- No need to submit to NHSBA
- Keep original for five years

What are the requirements for collection of a CD?
A
- Patient or patient rep must show ID unless they are known
- Healthcare professional must say name and address of HCP and give proof of identity
- If rep collecting for a drug misuse patient they must have a letter from patient giving consent with their name each time they come to collect and must show ID. Their details must be entered in CD register

A
- Make entry within 48 hours
- Keep records for 2 years

What is the purpose of running a balance?
A
- To identify irregularities and discrepencies that need to solved
- This should be notes and corrected
- It is an SOP and pharmacists responsibility to maintain running balances and dealing with discrepancies

Why may running balances for liquids be affected?

Overages, spillages, residue
