Reporting defective or isolated on-train equipment

You must stop your train at the first convenient opportunity and tell the train operators control when you become aware of a defect with the:
- Automatic warning system (AWS) - if not in operation on the train
- Doors unless they cannot be closed

- Sanding equipment - unless you believe you may have difficulty stopping the train if it continues in service

The driver of the train has a broken or obscured windscreen. As a competent person what must you help/advise the driver of?
You must help and advise the driver with:
- Sighting signals
- Speed restrictions
- Lineside signs
- Stations
- Level crossings
- Anything else the driver needs to be aware of.

If control of the automatic brake is no longer continuous throughout the train, what speed must you not exceed?
You must not exceed 5 mph (10 km/h).
You must also make sure, as often as possible, that the train is still complete.

The following doors are defective, locked or made inoperative. what are your actions?
Give the signaller:
- The vehicle number
- The location of the door
- The position of all door controls
- The position of the traction interlock switch at the time of the incident.
- And place the vehicle out of use

Appropriate action would be:
- Reducing speed
- Fequent use of the warning horn to ensure that the train or anyone working on or near the line, is not placed in any danger.
