Preconception bloods?
FBC, blood group and antibodies, rubella, hepatitis B, hepatitis C, syphilis, HIV
Recommended time period between live vaccines and conception?
1 month
Genetic carrier screening includes?
CF, spinal muscular atrophy and fragile X
Folic acid dosing and duration?
1 month prior to conception and for the first 3 months of pregnancy. 400mcg or 5mg where there is increased risk if NTD (anticonvulsant meds, pre-pregnancy DM and previous child or family history of NTD, BMI > 35
How’s does stress affect fertility?
Increased cortisol and prolactin levels directly affects the hypothalamus to reduce gonadotrophin release. Thie reduces FSH and LH from the pituitary deltaic preventing and reducing the quality of ovulation.
Iodine dosing
150mcg daily while pregnant and breastfeeding - all women
Vitamin A
Cease all synthetic derivatives of vitamin A - increases the risk of miscarriage and CNS malformations
Mercury containing fish
Avoid fish containing high levels of mercury and eat 2-3 serves of other types of fish per week - increase risk on foetal brain and CNS
Caffeine
Limit to 2-3 standard cups of coffee per day - increases risk of FGR
Recommended exercise
150 mins per week or 30 mins per day on most days - moderate intensity
Who is at risk of vitamin B12 deficiency?
Vegetarians and vegans
What are the effects of maternal B12 deficiency?
Neurological sequelae in exclusively breastfed infants
Risk factors for vitamin D deficiency?
Reduced sun exposure, veiled women, dark skin, BMI > 30
What are the effects of maternal vitamin D deficiency?
Low neonatal vitamin D –> impaired skeletal development and hypocalcaemic seizures
Recommended dietary calcium intake in pregnancy?
1300mg (aged 14-18yrs) and 1000mg (aged 19-50yrs)
