Q

Should all documents be placed under one incident #?

A

yes, example is a house fire with a burn patient. all information would be assigned to one incident #. a needle stick would be a second incident # because it was an incident separate from the initial incident. Or a fire truck returning from a fire that was involved in an accident would get 2nd incident #.
