when must an employee complete and submit appropriate electronic forms reporting injury, exposure, vehicle accidents, equipment and or property damage?
by the end of their shift.
if the employee is incapacitated or unable to complete the forms who’s responsibility does it become?
the immediate supervisor prior to the end of their shift.
when will investigators conduct an incident investigation?
any report of work related incidents resulting in accident, injury, exposure, or equipment property damage.
who gets the completed investigation report?
it will be submitted to the safety officer for review.
when should you notify the safety officer of any significant incidents?
immediately and they can help with the investigation as needed.
who gets involved when there is a fatality involving fire personnel?
law enforcement to completed a full death investigation
what is the responsibility of the involved employee?
notify supervisor

what is the responsibility of immediate supervisor?
notify the on duty BC
complete appropriate online reporting forms if injured employee in unable to do so by end of shift.
what is the battalion chief and HRs responsibility?
BC is in charge of shift and HR is in charge of day shift personnel. they need to call lnI to report any in patient overnight hospitalization or possiible fatal. the report must be made within 8 hours of the incident or 8 hours of learning of it.
what is LnI phone number?

a report to LnI needs to include what?
name of organization, location, time, and date of incident, # of fatalities, injured employees of pesticide exposures. and a brief description of the incident.
describe what the officer in charge does in the event of occupational injury, illness or exposure:
send the injured person to the medical facility if needed. complete a EPCR report if they are a patient. refrain from making public statements unless cleared by chief and follow appropriate routing for exposure kits.
who pays for leoff 1 injuries/ illnesss
they present their medical card for cost incurred. they are not covered under LnI insurance.
who pays of leoff 2 injuries?
notify the hospital that its an LnI injury and fill out the claim form at the medical facility.
who pays for volunteer injuries?
advise the medical facility it doesnt fall under LnI and to send all bills to the fire district which will be presented to the state board of volunteer firefighters for payment.
who pays for support service volunteers and chaplains?
they are not covered by the board of volunteer firefighters however the dist has decided to cover them under LnI. they are to open LnI claim at the medical facility.
what is an OSHA 300 log and where is it kept?
state law requires a log be kept at all stations on the safety bulletin board for the required time period. the 300 log tracks injuries illness and exposures of our employees.
what do you do if involved in a vehicle accident with injuries or significant damage?
notify dispatch of the situation, dispatch a BC, assess scene safety and check for injuries, request resources, notify the shop to eval damage, drug and etoh testing will be conducted if there is fatality, serious injury, or significant damage per drug and alcohol testing policy. 210
what do you do involved in a vehicle accident with minor damage or injury?
notify dispatch youre out of service, notify supervisor of situation, notify shop to assess damage.
what do you do if involved with equipment or property damage?
notify immediate supervisor, notify BC and if resources are needed to mitigate the situation contact A/C of logistics.
what does prompt accurate reporting and investigation protect the dept and employee form?
liability, false and unrealistic claims.
what happens if wrongdoing or gross negligence are found during an investigation?
the police department must be notified and also the safety officer notified immediately.
if the incident results or is expected to result in a fatality what should not be done?
you shouldnt move the vehicle or scene until LnI investigates the scene unless required to prevent further incidents or further injury.
who does the A shift investigator investigate?
A shift people, training division, 60 day shift members
