When to check QC
-Before 1” patient of the day
- After cleaning of battery change
- Changing of lot
- When the result not tally with patient condition
- When meter drop
- When the test strip bottle was left uncapped
High alert value?
- < 4 / >20mmol/L for patient not on SCSI
>24mmol/L for patient on SCSI
-< 2.5 / >25mmol (to do blood venous test) - (HI) indicate more than 33.3mmol
- (LO) indicate less than 0.6mmol
Hypoglycemia protocol
IF patient is conscious and able to swallow
(including tube feeding)
-Give 1 sachet of 15gram glucose in 120mls water STAT. Check BGM after 15mins,
-If BGM >4mmol/L give snack if meal is not within 1 hour and patient able to take orally.
If patient unable to orally, consider IV D5% or D10% at 50-100ml/hour
-BGM is <4mmol/L, inform Doctor and repeat 1 sachet of 15gram glucose in 120mls water
If patient is altered consciousness, unable to swallow or NBM
-Set IV cannula
-Prepare for Dr to give 40mls of IV Dextrose
50% as IV push STAT by Dr
-If Dr delay of 5mins or more is expected, nurse to initiate IV Dextrose 10% 500mls at 250mls/hr if Dr
* Has prescribed Dextrose 10% in EPIC or
Is not able to prescribe the Dextrose 10% in EPIC, nurse to initiate first and Dr to review patient ASAP within 30mins.
- IV Dextrose 10% is stopped after IV
Dextrose 50% is given by Dr or BGM
›4mmol/L.
- If patient’s venous access cannot be obtained, consider IM Glucagon 1mg stat if ordered
