A

1.Always ask patients for 2 identifiers
2.Do not send personal data to personal email
3. Password protect documents with personal data. Do not send password in same email.
4. Ensure email recipients are correct before sending correct before sending documents.
5. Do not lose your corporate device (laptop, phone)

Data breach incident happen (potential or actual)

A

-Staff is responsible to report to his/her RO and HOD immediately
-RO or HOD shall provide an interim update in DPO or PDPO either a phone call or secure text messaging chat or email immediately.
-DO or designates shall provide guidance in the steps to contain the breach and manage the incident, when required.
–Staff involved shall report the incident via PRISM.

A

-List in YH portal last updated was 10 years ago
-To search for interpreter list, enter the language and you will able to see the staff lists who can speak that certain language and it will flows from HR as staff indicate language spoken in hiring,

Pathways are all hardcopy, how are the data collected and transcribed?

A

Identified indicators (of the pathway) are collated manually, and there is plan to convert all pathway to electronic (in EPIC) which will take time
Note: Previously, case managers collated pathway data but no more case managers in KTPH.

What was done ff the incidences?

A

-SWI review is every 3 yrs and update it as necessary when new processes are introduced. Email will be sent to all staff when there is policy revisions.

Clinical quality board updated by OIC daily and shared during rollcall
-BSC will show all indicators performance of the month and shared during rollcall (the sharing is not daily, roll call one week upon displayed is sufficient however some KTPH ward practices all staff signed and read BSC.
