A

Components include; name of procedure, completion of instruments, sponge, swab/needle counts, labelling of specimens, any equipment problems, deposition of patient after surgery.

A

A

For blood transfusion consent: 1 month validity.

-What if patient is unable to sign or without limbs, how he/she can give consent?

A

-Thumbprint is a valid signature, the right thumbprint should be used unless the right thumbprint is missing hence must use the left thumbprint.
-For patient without upper limbs, he/she can give verbal consent after the doctor has given the appropriate information to make an informed consent

Who can sign the blood transfusion consent

A

For capable of giving consents: MO and above can sign

For incapable of giving consents;
Situations;
-Immediate life-threatening, emergency situation, elective transfusions: single signature from a doctor of registrar grade and above.

Who can sign for operation and procedure consent?

A

-For incapable of giving consents;
Situations;
-Immediate life-threatening single signature from a doctor of registrar grade and above
-Emergency situation(p2-p3 cases) the attending specialist (assoc consultant and above) can decide for the procedure however if specialist not on site, a doctor of registrar and above can sign the consent.

Elective -consent must be obtained from legal guardian. If cannot be obtained from legal guardian. 2 doctors of AC grade and above. One whom not be directly involve in patient’s care.
