COPD patients
Smokers
organisms:
typical:
strep pneumoniae
haemophilus influenzae
Atypical:
mycoplasma pneumoniae
Legionella
Chlamydophila pneumoniae
therapy:
Benzyl penicillin
azithromycin
stat dose of gentamicin
Hospital acquired pneumonia
most significant risk factor is intubation
findings consistent with pneumonia after patient has come to hospital or recently left hospital
nursing homes are treated as clinical settings
mild disease:
amoxycillin and clavulonic acid
moderate disease:
ceftriaxone
severe disease:
tazobactam and pipracillin
consider adding metronidazole for anaerobic cover
aspiration pneumonia
benzyl- penicillin or ceftriaxone based on severity
addition of metronidazole to cover anaerobes
