what is pneumonia ?
inflammation of lung tissue, usually due to infection
what are the 4 types of pneumonia ?
community acquired pneumonia
hospital acquired pneumonia
aspiration pneumonia
atypical pneumonia
what bacteria causes (CAP) ?
developed outside of hospital
streptococcus pneumoniae
what bacteria causes (HAP) ?
developed more than 48 h after hospital admission
gram -ve bacteria, MRSA, H. influenza, Klebsiella
what bacteria causes aspiration pneumonia ?
GI bacteria e.g. enterococcus
who does aspiration pneumonia effect ?
elderly, alcoholic, unconscious
what bacteria causes atypical pneumonia ?
chlamydia psittaci - from birds
pneumocystis jirovecci
legionella - through water
mycoplasma
what would you do for a community acquired pneumonia investigation ?
CURB - 65
explain CURB - 65
C - onset of confusion
U - urea > 7
R - resp rate > 30/min
B - BP systolic <90 or <60
65 - age 65 years or older
what would you do with your CURB -65 score ?
0-1 - investigation not necessary
1-2 - CXR, full blood count,
3 > - blood culture and sputum culture
what antibiotics do you give for community acquired pneumonia ?
non severe: amoxicillin (doxycycline if allergic)
severe: co-amoxiclav and doxycycline
what antibiotics do you give to Hospital acquired pneumonia ?
non severe: amoxicillin (doxycycline if allergic)
severe: amoxicillin and gentamicin
what antibiotics do you give for aspiration pneumonia ?
non severe: amoxicillin and metronidazole
severe: amoxicillin, metronidazinole and gentamicin
what antibiotics do you not give for legionella (3) ?
doxycycline, levofloxacin and clairthromycin
