What is the first drug used in lupus?

What are the commonly used DMARDs
Methotrexate, sulphasalazine, Leuflunomide, hydroxychloroquine
What kind of antagonist is methotrexate?
Folate, have to be on folic acid
What are the adverse effects of methotrexate?
Leucopenia/thrombocytopenia Hepatitis – alcohol intake must be limited Pneumonitis Rash/ Mouth ulcer Nausea/Diarrhoea Needs monitoring – FBCs and LFTs
How long after stopping methotrexate do you have to wait before trying for a baby?
3 months
How long after stopping leflunomide do you have to wait before trying for a baby?
2 years, due to its very long half life
What can hydroxychloroquine cause?
Retinopathy
What are the adverse effects of sulfasalazine?
Nausea, Rash, mouth ulcers, neutropenia, hep, reversible oligozoospermia
Requires monitoring
What are the targets for biologics?
TNF, CD20 B cells, IL6, IL17, 12 and 23
What are the adverse effects of anti TNF
Infection risk (reactivate latent TB), Skin cancer
What are the adverse effects of steroids?
- Weight gain
- Muscle wasting
- Skin atrophy
- Osteoparosis
- Diabetes
- Hypertension
