What is Perthes disease?
Disruption of blood flow to the femoral head, causing avascular necrosis of the bone.
Who does Perthes disease typically affect?
Children 4-8
How much more common is Perthes in boys than in girls?
5x more common in boys than girls
What is the typical presentation of Perthes disease?
3 weeks of hip pain with a worsening limp
How can Perthes present?
- Pain in the hip or groin
- Limp
- Restricted hip movements
- There may be referred pain to the knee
What is the initial investigation for Perthes disease?
X-ray
What is the problem of using an x-ray for someone with Perthes?
It can be normal
What early sign can be seen on an x-ray of someone with Perthes disease?
Widening of the joint space
What late sign can be seen on an x-ray of someone with Perthes disease?
Decreased femoral head size
What can be seen on an MRI of someone with Perthes disease?
Reduced perfusion to the hip on MRI
What is the management of Perthes disease of someone under 6?
No intervention
What is the management of Perthes disease of someone over 6?
Surgical repair
What is the main complication of Perthes disease?
A soft and deformed femoral head, leading to early hip osteoarthritis.
