what is it
inflammation of pericardium
often occurs with myocarditis
what are symptoms
sharp, central chest pain - exacerbated by movement, respiration and lying down relieved by sitting forward may be referred to neck or shoulders fever fatigue nausea
what are signs
pericardial rub (best heard with diaphragm at lower left sternal edge at end of expiration with patient leaning forward)
what are causes
ECHO virus
bacteria (extension from elsewhere - pneumonia)
fungi - post transplant
TB (caseous material in sac)
how is it diagnosed
symptoms
auscultation
ECG
CXR, echo
what does it look like on ECG
concave upwards (saddle shaped) ST elevation must be differentiated from MI
how is it treated
bed rest
analgesia
oral NSAIDs
what are complications
cardiac tamponade
what is signs of this complication
raised JVP, hypotension, muffled heart sounds, pulsus paradoxus
what is treatment of this complication
pericardiocentisis if not resolved itself
