what is myocarditis
inflammation of cardiac muscle, commoner in younger people
what are symptoms
fever
chest pain
SOB
palpatations
what are signs
arrhythmias
cardiac failure
soft heart sounds, prominent 3rd HS, tachycardia
pericardial rub may be heard
what causes it
coxsackie A and B
adenoviral infection
non infectious cause - after rheumatic fever, after drug or SLE
how is it diagnosed
ECG (ST/T abnormalities)
CXR - cardiac enlargement
viral culture / PCR
Elevated CK, troponin
what is treatment
supportive - treat underlying cause
bed rest in acute phase - avoid athletics for 6mnths
treat complications - ACEI, B blockers, spironolactone, digoxin
what is complications
HF, MI/stroke, arrhythmias
