Peds Flashcards
Drugs with dec dosing in peds
Opioids
more TBW = higher Vd
Less muscle = inc duration
Dec hepatic function and GFR
Dec protein binding in neonates = higher free fraction
Drugs needing higher doses in peds
Propofol
Thiopental
Sux
Hypnotics
Rate of infusion of dextrose containing fluids in neonates
4-7mg/kg/min
10% dextrose = 100mg/ml
Dose of epi for IV injection detection of local
0.5mcg/kg up to 15mcg
Positive test dose = inc T-wave amplitude by 25%
Post- T and A hemorrhage timing
first 6 hours
Congenital heart lesions requiring PGE1 infusion
Pulmonary or tricuspid atresia - where there is no other pulmonary blood flow
Cause of hypoxemia and cardiovascular collapse with CDH
Pulmonary HTN and lung hypoplasia
- should be fixed before correction
Management of HLHS hypotension and inc SpO2 following induction
Likely due to dec in PVR from high FiO2 and hyperventilation of mask induction.
Ductal dependent for systemic blood flow = balance pulmonary and systemic BF.
Dec MV, dec FiO2, fluid bolus,
PFTs with scoliosis
Restrictive - proportionate dec in all volumes
Ebstein’s anomaly
Septal and posterior leaflets of tricuspid valve positioned down into RV with long, redundant anterior leaflet.
RV hypoplastic
