Treatments for:
vWD type 1
vWD type 2
1 = desmopressin
2 = try desmopressin unless type 2b = vWF factor concentrate
Calculate volume of blood to transfuse
Volume = (Hct-end - Hct-start) x EBV / 65
When to use exchange transfusion
Acute chest syndrome
Most common transfusion reaction
Febrile non-hemolytic
- donor leukocytes
Coagulation factors downregulated in sepsis
Anti-coagulants
- thrombomodulin
- endothielan protein receptor C
- APC
- protein S
Most number of uncross matched O- blood one can receive and go back to type specific
2
Anti-A and Anti-B in donor O- will lead to intravascular hemolysis of type specific blood.
More O- will just give hyperbilirubinemia
Who needs washed RBCs
IgA deficiency - previous anaphylactic reaction to transfusion
Indications for DDAVP
vWD
Hemophilia A
Uremia-induced platelet dysfunction
