Scoliosis:
Lateral curvature greater than 10 degrees, often associated with rotational deformity
Scoliosis exam:
Forward bend test
Scoliosis treatment (2):
- > 30 degrees = brace
- > 40-50 degrees = surgery
Club foot:
Idiopathic congenital food deformity, characterized by plantar flexion of ankle, adduction of heel, high arch, adduction of the forefoot
Club foot treatment:
Manipulation and casting immediately, surgery if still rigid at 3 years
Osgood schlatter:
Overuse injury causing apophyseal injury at tibial tubercle
Osgood Schlatter, xrays show:
Fragmentation of tubercle
Hip displasia:
Malformed hip socket and femoral head
Hip dysplasia exams (2):
- Barlow exam
- Ortolani exam
Hip dysplasia treatment (2):
- Pavlik harness to force relocation of hip
- Casting and surgery in older kids
Perthes disease:
Idiopathic osteonecrosis of femoral head, between 4 - 8 years old
Perthes disease exam findings:
Decreased internal rotation and abduction
Slipped capital femoral epiphysis:
Displacement of the femoral head through the physis usually during growth spurt
Slipped capital femoral epiphysis exam finding:
Loss of internal rotation with hip flexed
If slipped capital femoral epiphysis is bilateral:
Workup for endocrine abnormality
Tarsal coalition:
Abnormal connection between tarsal bones
Tarsal coalition on exam:
Restricted hindfoot movement
Tarsal coalition on x-ray:
Calcaneal navicular coalition
Transient synovitis of the hip:
Sterile effusion of the hip causing pain
Transient synovitis of the hip test results (4):
- X-rays show effusion
- CBC, ESR, CRP not elevated
- Joint aspirate < 50,000 WBCs
- Gets better with NSAIDS
Most common elbow fx:
Supracondylar humerus fracture
Rickets:
Error in Vit D metabolism causes failure of mineraliztion
Rickets on X-ray (2):
- Looser lines (osteoid seam)
- Widened growth plates
Majority of upper limb growth from:
Ends of humerus and radius
Majority of lower limb growth from:
Knee
Common patterns of fx in non-accidental trauma (3):
- Posterior rib fx
- Spiral fx
- Metaphyseal corner fx
Cerebral palsy:
Permanent, non-progressive brain injury in the peripartum period
Achondroplasia (4):
- Disproportionate short stature
- FGFR3 mutation
- Autosomal dominant
- Hypertrophic zone in growth plate in narrow
Marfan syndrome:
Fibrillin mutation
Osteogenesis inperfecta:
Defect in collagen I from osteoblasts
