Pyloromyotomy

A

Curvilinear incision above the umbilicus, grab the pylorus and open the serosa and muscle just proximal to the pyloric vein on the duodenum, and extend that incision onto the antrum.
Once mucosa bulging out, complete a leak test by asking the anesthesiologist to inject some air while the pylorus is submerged under saline to look for bubbles.

Ped appendectomy

A

Use shorter trocars/instruments, lower insufflation pressure. Be wary of more intraperitoneal bladder and larger liver. Use hasson entry

A

Detorse bowel (counter clockwise)
Divide Ladds bands
Broaden mesenteric base
Appendectomy
SB in RUQ, colon in LLQ
