1. Do you have any pain or discomfort in your chest? If so, OPQRST/RSVP3/CHLORIDE AAA.
2. Are you short of breath during physical activity/what lead up to this event?
3. Do you sometimes get a fluttering feeling in your chest?
4. Do you ever feel faint?
5. Do you have any medical conditions? How is your blood pressure generally?
6. Are you taking any medications? Are you under a physician’s care?
7. Are you a smoker? Quantify in pack years.
8. Do you drink alcohol? Quantify.

9 .Do you have any family history of heart problems?
