11.3.1 Mandatory Occurrence Reports (MOR)

A

Required to be submitted to HKCAD within 96 hours of the reportable event occurring, made in Air Safety Report (ASR) Form.

Preferred method is through Corporate Safety and Quality (CSQ) who assess all safety reports received and submit those that qualify as reportable occurrences. However, it is the right of any pilot to insist that his / her report be upgraded and submitted as a MOR.

11.3.2 MOR Reportable Incidents

A

In-flight emergency requiring the use of emergency procedures or equipment.

Declaration of an emergency; “MAYDAY” or “PAN PAN”.

Failure of emergency or redundant equipment / systems to operate correctly.

Ground proximity warning in IMC Conditions. (NOT nuisance warnings)

TCAS Resolution Advisory (RA) where a REALISTIC THREAT of collision was deemed to have existed.

Activation of primary flight control warning system; overspeed etc.

Significant, unintentional deviation from assigned track or altitude (+/- 300’).

Significant inadvertent reduction in airspeed.

Flame-out, shutdown or significant malfunction of any engine.

Rejected takeoff at speeds near v1, which MAY resulted in a HAZARDOUS or POTENTIALLY HAZARDOUS situation.

Flap or Slat Asymmetry.

SIGNIFICANT loss of braking.

Warning of smoke or fire resulting FROM A SITUATION HAZARDOUS to the aircraft or its occupants.

ANY Significant damage to the aircraft howsoever caused.

Leakage of fuel or other fluids, which RESULTED IN A FIRE or CONTAMINATION hazard.

SERIOUS injury caused to any person during the operation of the aircraft (flight or ground).

Incapacitation of FLIGHT DECK OPERATING CREW.

Incapacitation of CABIN CREW whereby they are UNABLE TO PERFORM essential EMERGENCY DUTIES.

Passenger tampering (modify) with lavatory smoke detection equipment.

Passenger behaviour resulting in the use of RESTRAINING FORCE or use of the RESTRAINING KIT.

ABNORMAL VIBRATION and / or FLIGHT CONTROL VIBRATION felt by the crew.

A SEVERE turbulence encounter or an event deemed to require a “Turbulence Check” of the aircraft.

Turbulence encounters resulting in injury to passengers or crew.

11.3.3 Air Safety Reports (ASR)

A

Submitted within 24 hours of the event.

Crew are encouraged to report ALL safety related events that are NOT reportable under CAD 382. If in the light of further investigation, any report is deemed to be more serious it will be upgraded to a MOR and the reporter will be notified accordingly. If a persistent “low risk” event is flagged by the submission of multiple similar reports, they may also be upgraded to an MOR.

11.3.3 ASR Reportable Incidents

A

Whenever ATC notify the Commander / PIC the aircraft position is in error.

If a flight deviates by 300’ OR MORE from a cleared flight level while in RVSM Airspace.

If Wake Turbulence is encountered while in RVSM Airspace.

ANY TIME there is cause for a crewmember to enter the Avionics Bay during flight.

ANY occurrence of smoke or abnormal smell.

ALL Hard or Overweight Landings.

ALL incidents involving Dangerous Goods.

Injuries to CABIN CREW or PASSENGERS resulting from inflight turbulence (Convective Turbulence, Clear Air Turbulence, Mountain Wave Turbulence or Wake Turbulence).

CONFIRMED or SUSPECTED electromagnetic interference (EMI) event.

11.3.4 Other Safety Reports

A

Cockpit Crew Fatigue (CFR) is used by cockpit crew to report fatigue.

Quality Discrepancy Report (QDR) is used by cockpit crew to report non-safety related occurrences or concerns, such as incorrect procedures, suggestions for improvements or to report events NOT covered by ASR, CSR or GSR reporting programmes.

Volcanic Activity Report (VAR) is used by cockpit crew to report volcanic activity or a volcanic ash encounter.

11.3.5 Pilot in Command Reporting Responsibilities

A

In the event of an accident or serious incident resulting in injury, death or substantial aircraft, the PIC is responsible for notifying the nearest authority, by the quickest available means.

The PIC will:

Notify the appropriate local authority and / or appropriate state regulatory authority without delay in the event of any emergency situation that necessitated action in violation of local regulations and / or procedures.

Submit, if required by the State of Occurrence, a report the the appropriate local authority or appropriate state regulatory authority.

In practice, the PIC should leave a completed ASR form with the appropriate authority and transmit a copy of the ASR to CSQ by the quickest available means.

11.3.6 Specific Category Reports

A

AIRPROX (Including TCAS RA), Windshear / Turbulence and Birdstrike occurrence are normally made on specific forms by HKCAD. However ASR is also to an acceptable standard of CAD.

VAR is treated as a type of SAR.

Runway Incursions are reported in ASR and HCSQ will complete DCA 235 on behalf of reporter after consultation.

11.3.7 Confidential Safety Reporting (SCR), Flight Data Analysis Program Confidential Crew Feedback Report (FDF)

A

SCR: Preferably submitted electronically on the Dragonet CSQ website.

FDF: Provide additional information, confidentially, for occurrences that may subsequently feature in the bi-monthly Flight Data Analysis Program processes. Preferably submitted electronically on the Dragonet CSQ Website.

11.3.8 ASR / MOR Reporting - Aircraft Maintenance Log (AML) Entries

A

Whenever any technical deficiencies with aircraft components with ASR / MOR submitted, maintenance log entries are annotated with “ASR / MOR Raised”.

11.4 Company Accident Investigation Team (CAIT)

A

Unlikely will conduct primary investigation. Usually undertaken by State of Occurrence, State of Registry or the State of the Operator.

Main purpose is to provide assistance to the official investigatory authority.

Ensure timely information is available to the Company to prevent a re-occurrence.
