Beginning of symptoms
resting “pill-rolling” tremor in one hand
Cardinal signs
Tremor, rigidity, resistance to PROM, akinesia, postural instability, shuffled gait, falling backwards or forwards, mask face, micrographia
diagnostic testing
Degeneration in dopaminergic pathways in the basal ganglia, primarily in substantia nigra
Stage 1
Unilateral symptoms
Resting tremor
No or minimal loss of function
Stage 2
Bilateral symptoms
Problems with trunk mobility and postural reflexes; balance not affected
Stage 3
Impaired balance
Mild to mod impairment in function
Stage 4
Decrease in postural stability, function,
Mobility, need for assistance in ADLs, poor fine motor and dexterity
Stage 5
Total dependence for
Mobility and ADLs
