What is the function of parathyroid hormone?
Increase plasma calcium
What is the target tissue of PTH?
Bone, intestine, kidneys
How does PTH affect bone?
Releases calcium and phosphate
Stimulates osteoclasts
How does PTH affect the kidneys?
Stimulates calcium resorption
Stimulates phosphate and bicarbonate excretion
Stimulates conversion of inactive Vitamin D into calcitriol (active vitamin D)
How does PTH affect the GI/intestines?
Indirectly stimulates calcium and phosphate absorption in the intestines
Calcitriol stimulates intestines to absorb Ca/PO4
What stimulates PTH?
Small decrease in plasma calcium
What inhibits PTH?
Increase in plasma calcium
What are causes of hyperparathyroidism? Which is more common?
Adenomas (most common cause)
Hyperplasia of 2+ parathyroid glands
What are common lab findings of hyperparathyroidism?
Hypercalcemia
Hypercalciuria
Hypophosphatemia
Metabolic acidosis
What are S/S of hyperparathyroidism?
Kidney stones
Osteoporosis
Weakness, lethargy
Polyuria, nocturia, polydipsia
What causes hypoparathyroidism?
Surgical removal or damage of parathyroids
What are S/S of hypoparathyroidism
Muscle spasms, tetany, neuromuscular excitability, pruritis
What are lab findings of hypoparathyroidism?
Hypocalcemia
EKG arrhythmias
Hyperphosphatemia
Chvostek’s sign
