define pancreatitis

A

acute or chronic inflammation of the pancreas with elevated pancreatic enzymes

Etiology of pancreatitis

A

1 cause is gallstones where the gallstones blocks the opening to the duodenum. This block causes fluid to back up in the pancreatic duct and bile duct which leads to pain and inflammation.

  • Toxins
  • ETOH
    -hypertrigyceridemia
  • ERCP
  • Drugs

Presentation of pancreatitis

A
  • Sudden onset of constant sharp abdominal pain that radiates to the back
  • nausea
  • vomiting
  • severe SOB

A
  • CBC
  • LFT’s
    -Lipase (pancreatic enzyme)
  • CT A&P w/ IV contrast (Initial scan)

Management

A

Rehydration (Lactated Ringers) 5-10 ml/kg/hr
- Treat underlying cause (ex. gallstones)
- No antibiotics unless not improving after 48 hrs (Imipenem, Amoxifoxacin)
