IBD is divided in 2 diseases known as
- Ulcerative colitis
- Crohn’s disease
ulcerative colitis presentation
typically presents w/ bloody diarrhea
> 6 Bus per day
Lower Abdominal cramping
Bowel urgency
cramping rectal pain
Crohn’s disease can present with ….
RLQ ABD pain
some bloody diarrhea
fever
malaise
weightloss
Diagnostic for IBD
Endoscopy (especially UC)
CT and MRIs (Better for CD)
UC treatment
steroids
Treatment of moderate to severe UC flare
Screen for C diff
If negative: intravenous methylprednisolone doses of 40–60 mg/d or equivalent are the mainstay of therapy.
Severe pain and or high fever order
CT scanning to rule out acute abdomen and need for immediate intervention:
The following complictations require immediate intervention:
Obstruction
Perforation
Mega colon
Abscess

IBD w/u
CBC-may see anemia (malabsorption, bleeding, chronic disease), elevated plts or wbc
CMP-electrolyte imbalances, low albumin, elevated liver studies
CRP-inflammation
ESR-inflammation
IRON STUDIES-evaluate for deficiency related to bleeding or malabsorption
B12-malabsorption
STOOL STUDIES-Rule out infection
FECAL CALPROTECTIN-inflammatory marker in stool. May be normal or elevated
GOLD STANDARD FOR DIAGNOSIS IS ENDOSCOPIC EVALUATION AND PATHOLOGY
IBD + Cdiff treat with
oral vancomycin or fidaxomicin.
