what is it
reduced exocrine secretion of pancreatic leads to malnutrition maldigestion and gi symptoms
who it effects
CF, coelic , Zollinger Ellison, chronic pancreatitis, pancreatic/ gi resection pancreatic tumours
advice
1)avoid reduced fat diets
2) avoid alcohol
3) avoid hard to digests foods i.e. legumes (peas, beans, and lentils, high fibre diet
4) have 3 main meals with fibre across all these and 2-3 snacks
treatment
pancreatic (creon 3500)
- pancreatin supplement help with digestion starch , fat protein
age for pancreatin
c/I for <15 with CF CAUSE strictures
how take pancreatin
take with meals/snacks as it gets inactivated by gastric acid, can take immediately before or after
- if symptoms despite high pancreatic dose, add PPI acid suppressor
s/e
1) GI effects common
2) fibrosing colonopathy in high dose pancreatin risks; male, more severe cf, laxative use
3)irritate; perioral skin, buccal mucosa
excessive use: hyperuricaemia, hyperuricosuria, perianal irritate
