swelling of anal cushion containing blood vessels, inside snd outside anus
Symptoms
P B S I
pain after defecation (external piles)
Blood in stool bright red
swelling/ lump around anus- haemorrhoid
itchy sore skin around anus
risk factors
pregnancy, consitpation
advice
perianal hygiene, inc h20 and fibre
Treatment 3 stages
Constipation: bulk laxative
pain: simple analgesia (avoid opioid and NSAID in rectal bleed)
topical preparations with: local anaesthetic corticosteroid , astringent lubricant antiseptic
types of topical preparation
local pain
perianal inflammation
1)for local pain - local anaesthetic; lidocaine cinchocaine , primocaine
2)perianal inflammation- local corticosteroid i.e fludrocortisone, hydrocortisone , prednisolone exclude infection HSV, perianal thrush
