What is crohns
Inflammation GI tract from mouth to anus
Symptoms
Abdominal pain , diarrhoea , rectal bleed, weight loss, low grade fever , fatigue
Rotate flare up and remission
Complications
Abcesseces strictures fistula
Malnutrition, anemia
Colorectal cancer, small bowel cancers
Secondary osteoporosis
Growth failure in pubertal kids
Arthritis inflammation eyes joints skin, liver problem
Lifestyle
High fibre diet
Loperamide and codeine -can be used crohns not in colitis
Smoking cessation reduce relapse
1+ acute flare up in 12 months
Treat corticosteroid (pred, methyl pred, iv hydrocortisone)
Alternative budesonide or aminosalicylate in distal ileus, ileocaecal, right sided
2+flare up 12 months or if steroid dose can’t be reduced
Oral azathioprine or oral mercaptopurine
Alternative; MTX
If can’t tolerate either and Mab is applicable give Mab
Maintenance of remission
Oral azathiprine, mercaptopurine
Alt; MTX
Maintenance of remission after surgery
Azathiprine or mercaptopurine or aminosalicylate
