Mx of agitations and confusion in pal care?
Treat underlying cause as appropriate
Pharmacological - Haloperidol 1st line - also chlorpromazine + levopromazine
Terminal phase - midazolam
respiratory secretions/bowel colic: in pal care what to use?
hyoscine hydrobromide, hyoscine butylbromide, or glycopyrronium bromide
Painful mouth in EOL - mx?
Benzydamine hydrochloride mouthwash or spray
Severe renal impairment
What is the analgesia of choice and why?
Buprenorphine or fentanyl - not renally excreted hence less likely to cause toxicitiy
Oral morphine Subcutaneous diamorphine conversion?
divide by 3
Oral oxycodone Subcutaneous diamorphine conversion?
Divide by 1.5
Oral morphine Subcutaneous morphine conversion?
divide by 2
Oral codeine Oral morphine conversion?
Divide by 10
Oral tramadol Oral morphine conversion?
Divide by 10
Oral morphine Oral oxycodone conversion?
Divide by 1.5-2***
Mx of metastatic bone pain?
analgesia, bisphosphonates or radiotherapy
Mx of Hiccups in palliative care?
chlorpromazine is licensed for the treatment of intractable hiccups
haloperidol, gabapentin are also used
dexamethasone is also used, particularly if there are hepatic lesions
Mx of secretions in palliative care?
hyoscine hydrobromide or hyoscine butylbromide is generally used first-line
>the BNF does not suggest one is first-line over the other
>hyoscine butylbromide may be less sedative than hyoscine hydrobromide
glycopyrronium bromide may also be used
