First line of tx for RA (type of csDMARD)
Methotrexate (folic acid antagonist)- ONLY TO BE TAKEN ONCE PER WEEK
Cautions of methotrexate
Hepatic- toxicity
Children- specialist treatment
Pregnancy- avoid
Breastfeeding- avoid
Renal- not for mod-severe impairment
Second line of tx for RA (type of bDMARD)
Infliximab (TNF-alpha antagonist)
- IV only
- Taken with antihistamine/corticosteroid to reduce severity of ADRs
- Can take up to 12 weeks for effect
- Given every 6-8 weeks once maintenance dose reached
Cautions for infliximab
Hepatic- avoid
Children- avoid
Pregnancy- avoid
Breastfeeding- on specialist advice
