Work place violence reporting

*Can submit report directly to their supervisor
*Can submit it directly to the Workplace Violence Prevention Coordinator if they think their supervisor will not take the corrective action
*If there is a “privacy concern” do not write victims name in the report. Write “privacy concern case” for victims name
*Forward report via the chain of command to the “workplace violence coordinator”
*The workplace violence coordinator is Mr Nelson at the OSHA unit
After hours call FDOC
*Copy of workplace violence prevention policy is on the intranet. *Supervisors shall give a copy to employees when requested
*Hard copy of the workplace violence policy can be obtained from the OSHA unit
