What is osgood schlatters
Common cause of anterior knee pain in the skeletally immature athelete population. Tenderness at the patellar tendon insertion site and at the tibial tuberosity
Epidemiology and risk factors of osgood schlatters
Age- adolescent, 10-15 during puberty
Gender, more prevelance in males but gap is becoming smaller
Activity level, increased prevelance with the amount of sport and type of sport
Growth spurts, when bone grows faster than muscles and tendons
Symptoms in 20-30% of the population
Clinical presentation of osgood schlatters
A traumatic, anterior knee pain
Knee pain, localised at the facet, patellar tendon of the knee
Swelling or bones prominence at the tibial tuberosity
Tenderness around tibial tuberosity
Pain with activity that may worsen
Stiffness at the knee joint
Elys test- right quadriceps
Resisted isometrics of the quads give pain
Systemic effects of osgood schlatters
Altered activity levels, may reduce participation
Psychological impact, chronic pain may alter mood and self esteem participating in active adolescence who may feel intimidated
Weigh gain
Sleep disturbances
Postural changes
Increased risk of further injuries dues to compensation
Pathology of osgood schlatters
Repetitive stress on the knee during physical activity. Repeated pulling on the patellar tendon on the tibial tuberosity leads to inflammation and irritation at the attachment point. In some cases, this can be ossification or formation of a honey prominence due to chronic irritation
Differential diagnosis of osgood schlatters
Patella tendinitis-“ jumpers knee” inflammation of patella tendon
Knee ligament injury - ACL,MCL,PCL,LCL can present with knee pain
Osteochondritis disease- bone underneath the knee dies due to lack of blood flow
Septic arthritis, infection in the joint that can cause acute pain and swelling
Fractures- stress fracture of avulsion around the knee can mimic symptoms
Fat pad syndrome
Treatment of osgood schlatters
Conservative management; rest compression etc
NSAIDs
Activity modification
Rest
Surgery
Bracing or taping
