What happens to employees that report late for scheduled appointments?
Employees will not be seen and will be cited appropriately
When can members report for afternoon sick call?
1300-1430 only with approval of the MSO or Clinic Liaison in his absence
When shall Members notify their OIC of their current duty status?
Prior to leaving the PFC

What are the two reasons appointments with the PFC may be rescheduled? What is required following the request?
Emergencies and unforeseen situations; Documentation supporting the request
What happens when any Department personnel who is being carried on POD fails to report for a scheduled appointment? How long?
Carried on their Own SL until appointment is rescheduled or instructions of the PFC are followed
Are members permitted to partake in outside employment while on SL or POD?
No
What 3 things do annual physicals consist of at the PFC?
Lab Tests; Drug Tests; Physical Exam and personal interview
How long must a member not take anything but water by mouth prior to a physical?
6 to 8 hours prior
What happens when an employee is unable to produce a second sample for urinalysis?
deemed “Refusal to Test” and shall be considered grounds for disciplinary actions up to and including termination
When shall members schedule their annual physical?
Within 2 months PRIOR to the month in which their examinations is to occur
Who should be notified when members schedule their physical examinations? Who shall it be reported to?
Company Officer or Supervisor….They shall report the same to their BFC
Who does the MSO forward a list of personnel whom failed to complete their physical to and how often?
Appropriate AFC and Quarterly
How are members reimbursed for the time partaking in their physical exams off-duty?
Hour for Hour, In Comp Time

Form 44 and No PHI in Company Journal
What is Form 44.4 Safety used for?
Injury Reporting to be completed and forwarded by the Company Officer within 24 hours
What transmissions shall occur when a member suffers an injury in POD?
Radio request for Transport Unit Only; Contact the FLO/ELO via telephone with Name of injured, unit, extent of injury/illness
Who does the FLO notify when a member is injured or ill in POD?
DFC-Ops
Who must the Company Officer notify of a POD Injury? What will they do?
SAFO; if significant injury, involves the use of equipment, PPE, or stems from violation of department policy report to the scene and conduct an interview with any injured members and witnesses
If the period of SL is 30 minutes or more; how much SL should be charged?
One Hour or Equivalent
How much SL is earned for All-Hazards Uniformed employees in the Ops Division? What will be earned in other divisions?
4.5 Hours per pay period; 4 hours per pay period

YES; immediate family member that has a contagious disease and requires the care of the employee

No. SL can only be charged to an accrued balance and cannot be charged in advance without approval

DC Standard Form 1199; FEMS Form 11.4; Special Report
What is the max hours of Advanced SL allowable for Uniformed All-Hazards Employees? Other Divisions?
Not to exceed 288 Hours; 240 Hours
What Form can be submitted to substitute SL as Annual Leave?
Form 11.5; can only submitted for the current pay period
What are the 4 reasons members should have a Fitness for Duty Physical?
- Prior to appointment 2. Periodic Basis 3. Question about an Employees continued capacity 4. Not in a Full Duty Status for 30 days or more
