Oral Hygiene Instruction Flashcards
Purpose of OHI
- to improve the outcome of non-surgical periodontal surgery (NSPT)
- to have the patient become responsible for oral care by using self-care practices/techniques demonstrated by the hygienist
- to promote a behavior change that would result in long term results of NSPT
6 Motivational Theories
- Reward Theory
- Goal Theory
- Internal Motivational Theory
- External Motivational Theory
- Disease Theory
- Skill Enhancement Theory
Reward Theory
people repeat behaviors that make them feel good and discontinue behaviors that make them feel bad
Goal Theory
goals give rise to actions, goals create tension and people work toward the goal to reduce that tension
Internal Motivational Theory
- undertake an activity for its own sake
- self-generated
- don’t need much motivation from the hygienist to attain desired goal
External Motivational Theory
- depend on feedback from the hygienist, praise or valuable results
- needs enthusiasm, show concern, try personalizing the message
- focus on chief concerns of patients
- help patients set short term goals from appt to appt
Motivational Model
self esteem (2) - confidence self esteem (1) - fame love and belonging safety needs physiological needs
Learning Ladder
habit action involvement self-interest awareness unawareness
Clinical Preventive Services
scaling and root debridement prophylaxis sealants fluoride varnish athletic mouthguards
Why should OHI be given before scaling?
time may be limited
sensitivity from treatment
possibility of disturbing healing process of treatment
patient may be tired, anxious to leave and less receptive to instruction
Stiffness if Bristle Diameter
- soft brush 0.2mm
- medium brush 0.3mm
- hard brush 0.4mm
Sonic Brushes
produce high frequency vibrations (1.6 MHz), which leads to cavitation and acoustic microstreaming; disrupts bacterial cell wall [bactericidal)
Ionic Brushes
change surface charge of tooth by an influx of positively charged ions, repelling plaque from tooth and attracting to the negatively charged bristles
Brushing Techniques
Stillmans –> recession
Charters –> open interdental spaces
Bass –> normal
Fones –> young children; teeth occluded, bristles in circular motion
Horizontal –> causes recession, no benefit
Embrasures
- Type I: healthy, gingiva fills interproximal area under contact area
- Type II: slight-mod recession of interdental papilla
- Type III: complete loss of interdental papilla, extensive recession
