Domestic Violence
- violence between adult and intimate partner
- largest cause of injury to women ages 15-45
- 35% of injuries to women in hospital com from domestic violence
Elder Abuse
- 70% age 60 and over
- physical, sexual, psychological abuse
- financial abuse, illegal use of funds
- neglect: withholding food, medication
- exploitation: illegal use of adults resources for another’s profit or advantage
What to do in suspected elder abuse?

HIPPA
allows health care provider to release medical records of individuals who are, or are suspected of being, victims of abuse
Rights of Mandated Reporters
- immunity from civil and criminal liability
- protection of identity
- permitted to release confidential information without penalty
Child Abuse
-many abusers are victims of child abuse themselves “learned behavior”

Problem
- 3 deaths/day in US
- average age of child abused 3yrs
- over 2 million cases reported each year
- 86% of abused children have injuries to the head and neck yet dentists and hygienists reported 1%
- abused children have 800% greater incidence of rampant caries
- in VA, dentists reported < 0.3% of all cases of child abuse
Common Peri- & Intra-Oral Indicators of Abuse
- bite marks (on tongue)
- torn frenum
- palatal bruising
- avulsed teeth
- “black eyes”
- damage to the ears
- intra- and extra-oral burn marks
Behavioral Indicators of Abuse
- no eye contact
- wary of parents
- fear of touch
- dramatic mood changes
- withdrawn
- aggressiveness
- running away
- suicide attempts
Typical Sites of Inflicted Injuries
-buttocks
-genitals and inner thighs
-head and neck
cheek slap
earlobe
upper lip and frenum
neck
Dental Neglect
willful failure of parent to seek and follow through the treatment necessary to ensure a level of oral health essential for adequate function and freedom from pain and infection
Documentation
-record clinical and behavioral findings in chart
-take radiographs of affected area
-take clinical photographs
-express concern and confirm person’s worth
give referrals
-report to proper agency
-name, address, phone number of victim
-name of parents
-suspected injuries
-disposition of situation when abused victim left office
Consequences of Not Reporting
- in non-fatal cases, 35% will be abused again within the year
- w/o intervention, 5% of victims of sever abuse will die from further abuse
