A

1) Discuss with patient
2) Empty bladder
3) Double set up
4) Adequate anesthesia
5) Head engaged
6) Cervix completely dilated
7) Membranes ruptured
8) Position of the fetal head determined
9) Adequate pelvis
10) EFW: 2000-4000g

Indications for operative vaginal delivery

A

1) Prolonged second stage
2) Suspicion of immediate or imminent fetal compromise
3) To shorten 2nd stage for maternal benefit

Contraindications to operative delivery?

A

1- fetal head is unengaged,
2- the position of the fetal head is unknown
3- fetus is known or strongly suspected to have a bone demineralization condition (eg, osteogenesis imperfecta) or a bleeding disorder (eg, alloimmune thrombocytopenia, hemophilia, or von Willebrand disease).
