How to give morphine if pt is already taking codeine and still the pain is not settled for cancer pain?
A
Dose of codeine per day/10 = Morphine dose
Eg, Pt is taking 240mg of codeine per day (60mg x 4). To convert to morphine, this figure needs to be divided by 10. This gives a daily dose of 24mg.
