A = 2U1B1d, simple transverse B = 2U1B1e[3a] transverse impacted C = oblique D = 2U1C3 comminuted E = oblique distal F = fracture dislocation
Indication for figure of 8 wire banding
A and B
C with screw and then banding
Indication for plating
D-E
Radial head fracture dislocation
ORIF is enough to stabilize
No need for UL reco
Treat with functional bracing post-operatively
If no ORIF possible
Prosthesis (only 50% of normal stability)
indication for conservative
undisplaced, enough periosteum to allow for extension
Terrible triad TTT
Reconstruct Radial head if possible
For coronoid, depends on Regan Morrey (2U1B2):
- I (apical 1/3) : no fixation
- II (middle 1/3) : check intraop dislocation tendency (Kocher, if needed ulnar approach)
- III (basal 1/3): fixation needed
Olecranon and Coronoideus fracture
Olecranon : plate
if possible, fixation of coronoid through the fracture
Olecranon, Coronoideus and radial head fracture
either through posterior fracture or through Boyd (recline anconeus)
Anterior transolecranon fracture-dislocation
Radial head usually intact
Posterior transolecranon fracture dislocation
Radial head usually fractured
Proximal Monteggia
actually not an elbow fracture dislocation but a radial head dslocation with olecranon fracture
