Q

What does a stablisation appliance do?

A

Reduces TMD symptoms

Reduces bruxism protecting the dentition (in the long term it can get worse)

Q

What is the purpose of anterior positioning appliances?

A

Can allow taking away clicking/locking caused by disc displacement

Allows for adaptation of the retrodiscal tissues

Reduces clicking in the short term and the long term

6
Q

What are the adverse effects of using anterior positioning devices?

A

Myostatic contraction of the inferior lateral pterygoid.

If this occurs it can be mitigated and prevented from getting worse

8
Q

Why are anterior bite appliances used?

A

No good evidence that it reduces TMD but some evidence.

Reduces headaches (New claim)

Stay away from these as often as possible

Q

What are the adverse effects of using anterior bite appliances?

A

Stabilisation is better for TMD

Potential for occlusal changes (Eruption of posterior teeth -> Anterior open bite)

Potential to allow condyles to seat to a new position in the fossae creating a malocclusion

Q

What are posterior bite appliances used for?

A

No real data supporting long term use but they are useful for acute disc displacement without reduction (leading to earlier callous formation)

11
Q

What are pivot appliances used for?

A

They create a high spot on the mandible causing mandible to pivot out of the fossa.

Q

What are the adverse effects of using resilient appliances?

A

Increases nocturnal bruxism (doesn’t necessarily increase symptoms though)

Q

What are hydrostatic appliances used for?

A

Potentially useful to allow bite in even fashion to relieve discomfort.

They don’t last very long but can be used as a diagnostic tool

16
Q

When are mouth restricting appliances used?

A

For patients who anteriorly subluxate (limits how far they open)

Training device to stop patient from opening too wide

17
Q

What are the adverse effects of mouth restricting devices?

A

Irritates soft tissue

Potential occlusal changes if left on too long

18
Q

What are the adverse outcomes that can arise with MADs?

A

Some occlusal changes and TMD symptoms can be aggravated
