NSAIDs Flashcards
Celecoxib
- Selective COX -2 inhibitor
- Reversible
- Treatment of A) RA
B) osteoarthritis
C) acute mild to moderate pain - Oral , once or twice daily, h.l 11 h , p450 , urine and feces
Acetaminophen
- Inhibits PG synthesis in the CNS
- Antipyretic and analgesic
- Less effect on the COX in peripheral t ( weak I )
- مهم خافض حرارة و مسكن للالم عند الاطفال المصابين بال chickenpox due to (the risk of raye syndrome with aspirin)
Inhibitions of COX-1
Prevention of CV events
Aspirin at high doses:
Exhibits Anti inflammatory activity
Aspirin at lower doses:
- Prevention of cardiovascular events such as stroke and myocardial infarction
- Analgesic activity
Aspirin
- Bind irreversible
- Inhibit platalet aggregation
- Is used acutely to reduce the risk of death in acute MI and in patients undergoing revascularization procedure
Analgesic action
- Inflammation ➡️histamine and pradykinin ➡️PGE2 ➡️ sensitize nerve ending (⬇️PGE2 >⬇️pain )
- ⬇️COX-2 ⬇️pain
Used for mild to moderate pain
Ketorolac
Can be used for severe pain but shot duration
Antipyretic action
Inflammation ➡️WBCs➡️cytokines ➡️PGE2 ➡️fever
❌PGE2:❌fever
Inflammations that can be treated by NSAIDs
- Osteoarthritis
- Gout
- Rheumatoid arthritis
Combination of opioids and NSAIDs is used for?
Treating pain caused by malignancy
NSAlDs that can be used for antipyretic activity
- Aspirin
- Ibuprofen
- Naproxen
Raye syndrome
A syndrome that can cause fulminating hepatitis with cerebral edema, often leading to death
How to treat chickenpox
Aspirin should be avoided ❌❌❌ in patients less than 20 years old with chickenpox or influenza
2. Acetaminophen ✅✅✅is the analgesic/ antipyretic of choice for children with chickenpox
Methyl salicylate
Is used as cutaneous counterirritant such arthritis cream and sports rubs
