NICU Flashcards
average age of kids in NICU
under 1 year
theory of synactive development
5 areas to observe: ANS attention/interaction basic states (sleepiness, alertness) motor self regulation all these things are to assess baby's abliity to come back to a self regulated state
motor
tone
should be doing what they would be doing in the womb
posturing
ANS
skin color
-does it change when u stimulate them, does it go back to normal
state
sleepy, drowsy, awake all the time, fussy,
attention/interaction
eye contact, blinking, etc
self regulatory
attempts to achieve and maintain balance in these subsystems.
when should a premie be able to do all this?
40 weeks, should see progress by 30 weeks but 40 weeks should be good.
signs of stress of neonate
color changes BP changes HR responses visceral signs (gas, vomiting, gagging, hiccups, yawning)
motor signs of stress
frantic flailing movment
fingers splay
hyperextension of limbs
state signs of stress
glassy eyed look gaze aversion staring panicked look irritability diffuse sleep (twitching, grimacing, not resting peacefully)
strategies for dealing with stress
> combine healthcare professional encounters, be there at the same time as the neurologist, or the nurse or whatever
move slowly
one stimulation at a time (don’t look at baby while feeding)
LOTS of parent education
neuronal growth theory
there are times when the neurons in the brain are at peak development and when they are pruning so PTs should target their therapies to when the specific neurons are at their peak
